What is your current occupation and how long have you been with your current employer?

Why have you considered adding a wolfdog to your family? Why do you feel they will be a good fit for you & your lifestyle?

Do you have a fenced in yard? If yes, please describe size of area fenced, fence type, and height?

Please provide a brief description of how your puppy will spend his/her days if you are approved?

Please descibe the general area that the puppy will spend most of their time.

How will you ensure your puppy is adequately socialized with both people and other animals?

How do you plan to approach housebreaking?

How do you plan to train your puppy in obedience?

Who will care for your pet(s) when you travel?

Please list the pet(s) that currently reside in your home.

Have you ever surrendered an animal to a shelter or rescue? If so, please explain why and when this took place.

Are you willing to have your puppy spayed/nutered and agree he/she will not be bred?

Are you wanting to purchase breeding rights with your puppy? If yes, please describe your previous experience breeding wolfdogs.

Please list any other pets you have owned within the last 5 years and explain the circumstances regarding why they are no longer with you.

Please list all members that reside in your household, including their ages.

Please list your primary veterinarians name, address, and phone. How long have you been a client at this practice?
